Trinity Graduate School (TGS) Forms

The Trinity University Records Office has provided the following forms for student use. You may download/print the forms and submit to the Records Office. All forms arealso available in the Records Office. If you have any questions about form usage, or problems accessing forms, please contact Records at 847.317.7050.

Academic Appeals

 Guidelines for Academic AppealsAcademic Procedure for submission of student appeals to the Trinity Graduate School Appeals Committee.

Capstone

 CAPSTONE ApprovalDocuments approval for Masters students to register for CAPSTONE and other independent study courses. [This form must be submitted with the appropriate registration form, signed by the instructor of record.]

Graduation

 Intent to GraduateInitiates the processes related to graduation and serves to inform Trinity of your plans for completion of all degree program requirements. [NOTE: Per TGS policy, filing of the Intent to Graduate form in the Records Office must be made one semester prior to the intended May, August, or December graduation date]

Registration

 Cross Registration Form (into Trinity College)Cross registration of full-time Masters students in a college course. [Students must meet the qualification for Cross Registration and have the required approvals before they will be registered.]
 Full Course Request Information FormSubmit to the Academic Dean's Office. Requests which will be reviewed
by the Academic Dean and Department Chairs in order to make seating
adjustments and add sections, as needed.
 Independent Study ApprovalDocuments approval for Masters students to register for Guided Research Courses, Thesis/Education Project, Integrative Paper/CAPSTONE, Reading Courses.  [This form must be submitted with the appropriate registration form, signed by the instructor of record.]
 Overload ApprovalDocuments approval for Masters student to register for more than 18 hours per the Academic Handbook. [Students must appeal and be granted such approval before being allowed to register for an overload.]
 Registration/Add-Drop Form

For registration outside of TIU Access. (Used by visiting students, auditors, and extension students or for registration of independent study, reading courses and capstones, submitted with proper authorization.)

 

Student Information

 Change of Address/Summer Address Notifies the Records Office of a temporary or permanent change to their contact information with the University.
 Change of Advisor (TEDS/TGS)Approval to change Academic Advisors. [In order to process this request the form must be signed by both the current and requested faculty advisor.]
 Course SubstitutionDocuments approval for a course substitute for program CORE requirements. [Approval must be obtained from the faculty advisor or subject department chair.]
 Student Directory Information ReleaseUsed to limit the access of Directory information by the school to outside parties.
 Student Information FormGives/Verifies student contact information, used by students who have not taken classes at Trinity before. [NOTE: This is especially necessary for Audit Only and Trinity Spouse registration.]
 Verification Letter Request (TEDS/TGS) Requests a letter of verification regarding student academic information. [Records is able to verify the following type of information for students: Graduation; Full-time/Part-time Status; Program Enrollment; Grades for Tuition Reimbursement; etc.]

Thesis

 Thesis: Deadline Extension ApprovalRequests an extension to any of the designated thesis deadlines up to, and including, the final (Defense) Draft.
 Thesis: Topic & Committee ApprovalApproval for Thesis Topic and Thesis Committee. [Thesis Committee: First and Second Reader.]

Transcripts

 Transcript Request (TEDS/TGS)Requests TEDS/TGS transcripts. (NOTE: A hard copy or faxed signature is required for the release of a transcript.)

Transfer Credit

 Transfer Credit Request Requests the evaluation of course work completed at a different school for credit toward their current Trinity degree program.
