Trinity College (TC) Forms

The Trinity University Records Office has provided the following forms for student use. You may download/print the forms and submit to the Records Office. All forms are also available in the Records Office. If you have any questions about form usage, or problems accessing forms, please contact Records at 847.317.7050.

IMPORTANT NOTE: The form will be valid only upon receipt by the Records Office.

Academic Appeals

  Guidelines for Academic Appeals College Catalog Procedure for submission of student appeals to the College Academic Appeals Committee (CAAC).

Academic Information

  Change of Advisor (TC) Approval to change Academic Advisors. [In order to process this request the form must be signed by both the current and requested faculty advisor.]
  Declaration of Major Form Declares your Major Field of Study. [In order to process this request you must have the signatures of your advisor and the Department Chair of the school of your major.]
  Declaration of Minor Form Declares your Minor Field of Study. [In order to process this request you must have the signatures of your advisor and the Department Chair of the school of your minor.]
  Intent to Graduate Initiates the processes related to graduation and serves to inform Trinity of your plans for completion of all degree program requirements. Must be filed one calendar year prior to the date (semester) that a student intends to complete all degree requirements.  
  Guidelines for Filing the Intent to Graduate Form Instructions for accurate filing of the Intent to Graduate form.
  Pre-approval of Transfer Credit Form Requests the evaluation of course work completed at a different school for credit toward their current Trinity degree program.
  Six-Hour Contract Based on College catalog policy, request for permission to participate in the ceremony for students who are within 6 hours of meeting graduation requirements.
  Substitution/Waiver Form Approval for: 1) a course already taken either at Trinity or at another institution to complete a Trinity catalog requirement; -or- 2) a Trinity catalog requirement to be waived. Department Chair and Advisor signatures are required.
  Verification Letter Request Requests a letter of verification regarding student academic information. [Records is able to verify the following type of information for students: Graduation; Full-time/Part-time Status; Program Enrollment; Grades for Tuition Reimbursement; etc.]

Registration

  Course Add/Drop Form Requests an Add or Drop of a Course from a students registration at Trinity College.
  Cross Registration Form (into TEDS/TGS) Cross registration of full-time College students in a TEDS/TGS course. [Students must meet the qualification for Cross Registration and have the required approvals before they will be registered.]

Student Information

  FERPA Request to Withhold Directory Information Every Trinity student has the right to withold Directory Information under FERPA. Filing this form will preclude Trinity International University from providing Direcotry Information to anyone without the student's express written consent, except as provided by law.
  Permanent Address Change Form Notifies Records of a change to a student's permanent address information.

Transcripts

  Transcript Request (TC) Requests Trinity College transcripts. (NOTE: A hard copy or faxed signature is required for the release of a transcript.)
