The web transcript request form in PDF format is available here. Complete instructions for transcript ordering and payment, as well as mailing addresses, is included in the request form. Mail or fax this request form to the Records Office.

Option #2

You may come to the Records Office and fill out a "Request for Transcript" form.

Important Notes about Transcript Processing

Transcript requests should be submitted to the Transcript Coordinator in the University Records Office.
